New to Rutherford County Schools

IMMUNIZATION FACTS

 

  • All new students: (NEW to Pre-K, Kindergarten, Tennessee, from a private school or homeschool setting) are required to show proof of vaccination on the “Tennessee Certificate of Immunization" (you can get vaccines transferred by your Primary Care Physician or the Rutherford County Health Department). This has to be done to register at any Rutherford County School. 
  • All new students must also show proof of a health PHYSICAL (completed within the last calendar year or within the 1st 30 days of school). This also needs to be on the Tennessee Certificate of Immunization (physical should be documented in the upper right corner of form).

 

The student can get the required vaccines at any pharmacy, walk in clinic, primary physician, or health department. The immunizations must be transferred to the “Tennessee Form” at the Doctor's Office or the Health Dept. (Sample form attached below is the ONLY form that can be accepted per Tennessee policy).

Exceptions will be granted to any child whose parent or guardian shall file with school authorities a signed, written statement that such measures conflict with one of the following:

  1. His/her religious tenets and practices if in the absence of an epidemic or immediate threat of an epidemic; or
  2. Due to medical reasons if such child has a written statement from his/her doctor excusing him from such immunization.

Rutherford County Health Department has made it easier to get transferred and updated immunization records. When submitting a request, please ensure that ALL REQUIRED fields have been completed. A clear copy of immunization and physical records will ensure a timely response. The response time could be up to 1-3 days. Please click the link below to transfer records to the TN form:
